waiver & release of liability: Arise Above Basketball Program

Effective Date: February, 2026

By registering for Arise Above, I (parent/guardian) acknowledge and agree:

Assumption of Risk

Participation in basketball and related activities involves inherent risks, including injury. I voluntarily assume all risks associated with participation.

Medical Consent

I authorize Arise Above staff to seek emergency medical treatment for my child if necessary.

Travel Consent

I give permission for my child to travel with Arise Above for tournaments, including out-of-state travel.

Release of Liability

I release and hold harmless Arise Above, its coaches, staff, and affiliates from any claims, injuries, damages, or liabilities arising from participation.

Responsibility

I understand that I am responsible for:

• Timely payments

• Ensuring my child follows team rules

• Providing accurate medical and contact information

By registering and/or making a payment, I acknowledge that I have read and agree to this waiver.
